Central venous line is one of a creative instrument that saves human’s life in critical medical situation. Central venous line access is frequently involved in the disease management. It is used for rapid fluid therapy, transvenous pacemakers, infusion of some medications, hemodialysis or plasmapheresis and etc. Most of the emergency departments have some staffs that are trained for central venous line insertion but related complications occur during central venous line placement.Central venous line might have some complications and complication follow-up should be considered. Thromboembolism and infection are two important medical complications. Arterial puncture, hematoma, pneumothorax and hemothorax are mechanical Central venous line complications. Chest X-ray and some other techniques should be used for detecting these complications.Central venous line tip misplace is a considerable problem for emergency department staffs, previously chest X-ray has been used for central venous line misplace detection. In some recent studies, contrast-enhanced ultrasonography and intravascular electrocardiography have been used for central venous line misplace.

Nucleophosmin1 is a multifunctional protein that shuttles between nucleus and cytoplasm in some subtypes of acute myeloid leukemias. Mutated Nucleophosmin1 expresses aberrantly in the cytoplasm of the cell and transports from nucleolus to the cytoplasm. It is diagnosed by immunohistochemical techniques, flow cytometry assay and mutational analysis. The aim of this study is to evaluate the effects of Nucleophosmin1 mutation on the clinical presentations, prognosis, diagnosis and the treatment of acute myeloid leukemia. Thirteen articles were extracted from PubMed, Google scholar and Scopus in which the Nucleophosmin1 mutation correlated with gingival hyperplasia, high white blood cell count, lymphadenopathy, high platelet count and other signs and symptoms of myelomonocytic and monocytic acute myeloid leukemias. This mutation is a provisional entity in the classification of acute myeloid leukemia, which influences on the prognosis, clinical course and the treatment of some subtypes of acute myeloid leukemias. Nucleophosmin1 mutation has favorable prognostic value in the absence of other concomitant mutations.

Hyperbilirubinemia is a common disease and unconjugated hyperbilirubinemia has been seen mainly in neonates. Severe form of unconjugated hyperbilirubinemia may cause kernicterus and even death. Conventional treatment for severe unconjugated hyperbilirubinemia consists of phototherapy and exchange transfusion that have several known disadvantages; specially exchange transfusion is associated with a significant morbidity and even mortality. These harmful effects indicate the need to develop alternative pharmacological treatment strategies for unconjugated hyperbilirubinemia. One of these pharmacological agents is zinc salts. Zinc has been shown to lower the bilirubin levels by inhibition of the enterohepatic cycling of unconjugated bilirubin. Oral zinc has been shown to reduce serum unconjugated bilirubin in animals, adolescents and low birth weight neonates. However, studies in healthy term neonates given oral zinc showed no reduction in hyperbilirubinemia based on daily measurement. In order to improve the accuracy, hyperbilirubinemia may be determined based on measurements every hour. More studies are needed to know the effect of zinc in neonatal jaundice.

Adipose tissue surrounding the heart may contribute in the progression of coronary atherosclerosis due to its proximity to the coronary arteries. In addition, epicardial adipose tissue has paracrine and endocrine functions. It can secrete numerous bioactive molecules. Most previous studies examined the relation between coronary artery disease and epicardial adipose tissue have used echocardiography and have reported controversial results, probably due to differences in measurement techniques and study populations. This study aimed to give a brief review on the value of echocardiographic assessment of epicardial adipose tissue in the prediction of coronary artery disease severity.Epicardial adipose tissue, easily and non-invasively evaluated by transthoracic echocardiography, can be considered as an adjunctive marker to classical risk factors despite all the limitations. Moreover, it might be recommended as a useful quantitative screening examination for the prediction of the presence and the severity of coronary artery disease and the extent of atherosclerosis.

Pain is the most common complaint in emergency department and there are several methods for its control. Among them, pharmaceutical methods are the most effective. Although intravenous morphine has been the most common choice for several years, it has some adverse effects. There are many researches about intravenous acetaminophen as an analgesic agent and it appears that it has good analgesic effects for various types of pain. We searched some electronic resources for clinical trials comparing analgesic effects of intravenous acetaminophen vs. intravenous morphine for acute pain treatment in emergency setting. In two clinical trials, the analgesic effect of intravenous acetaminophen has been compared with intravenous morphine for renal colic. The results revealed no significant difference between analgesic effects of two medications. Another clinical trial revealed that intravenous acetaminophen has acceptable analgesic effects on the post-cesarean section pain when combined with other analgesic medications. One study revealed that administration of intravenous acetaminophen compared to placebo before hysterectomy decreased consumption of morphine via patient-controlled analgesia pump and decreased the side effects. Similarly, another study revealed that the infusion of intravenous acetaminophen vs. placebo after orthopedic surgery decreased the consumption of morphine after the surgery. A clinical trial revealed intravenous acetaminophen provided a level of analgesia comparable to intravenous morphine in isolated limb trauma, while causing less side effects than morphine. It appears that intravenous acetaminophen has good analgesic effects for visceral, traumatic and postoperative pains compare with intravenous morphine.

Ischemic nephropathy is defined as a clinically significant progressive reduction in glomerular filtration rate that is usually associated with significant renal artery stenosis (unilateral or bilateral involvement). Atherosclerotic renal artery disease is known as the most common cause of the ischemic nephropathy. These patients may develop secondary hypertension. In addition, epidemiologic data has showed a clear association between atherosclerotic renal artery stenosis and coronary artery disease and other cardiovascular disease. Despite the preserving function of kidney on various autoregulation processes, unusual microvascular function will be resulted due to sustained decline in renal perfusion. The ischemic nephropathy of asymptomatic cases may result in renovascular hypertension and renal failure. The reduction of renal function in these patients might be decreased or stopped by early appropriate diagnosis and also might be treated with renal artery angioplasty or surgery, after medical management. There is a debate about the occurrence of ischemic nephropathy as a result of atherosclerotic renal artery stenosis. In this study we aimed to review the prevalence of ischemic nephropathy due to atherosclerotic renal artery stenosis.

Renal transplantation was considered as an efficient and ideal method for end stage renal disease treatment of the patients, by accomplishment of the first successful transplantation in 1954 (Boston USA). Renal transplantation has transmuted the treatment of choice in end stage renal disease, along with prolonging survival; it offers high quality with low morbidity. Imaging modalities play an important role in the diagnosis of complications arising in renal transplant. Color Doppler ultrasound is the first-line imaging modality for evaluation of renal graft. Computed tomography scan in parallel with magnetic resonance imaging and digital subtraction angiography are used as problem-solving tools in indetermination of cases. Interventional radiology such as transluminal angioplasty has an important role in management of complications. Use of real time ultrasound guidance for percutaneous biopsy is almost universal.

Today, radiological procedures using intravascular iodinated contrast media are being widely used for the diagnoses and treatment of various diseases, which highlight one of the main etiologies of contrast-induced nephropathy and hospital-acquired renal failure. Contrast-induced nephropathy development is associated with longer hospital stay, increase in both short- and long-term morbidity and mortality, in addition to greater health care costs. The pathogenesis of contrast-induced nephropathy has not yet been fully explained in detail; however it is clear that the root concept is medullary hypoxia-induced renal tubular damage.Chronic kidney disease and diabetes mellitus are the two most important intrinsic predisposing factors to contrast-induced nephropathy. As no treatment can specifically target contrast-induced nephropathy, the main goal for clinicians is prevention of the disease. While the best approach for achieving this goal is still controversial, optimization of the patients’ circulating volume remains the only proven strategy to date. As contrast-induced nephropathy is a potentially preventable clinical condition, its better understanding will lead to better prevention of this disease. Hereby, we aimed to discuss contrast-induced nephropathy from 7 different aspects in clinical practice: 1) clinical aspect, 2) prevalence, 3) pathophysiology, 4) contrast agents and renal cell apoptosis, 5) different contrast media, 6) prevention, and 7) treatment.

Attention deficit hyperactivity disorder is one of the most common psychiatric disorders in childhood. Around 25-50% of these children suffered from some kind of sleep disorder especially with chronic form of insomnia. The physicians usually have a plan for improving hyperactivity and attention deficit of this disease but unfortunately, they forget to manage the sleep disorders, which are a major part of patients’ problems.Nowadays, we know that there is a noticeable relationship between attention deficit hyperactivity disorder and sleep disorders and by improving these children’s sleep, not only the daily functions improve, but also the symptoms of attention deficit hyperactivity disorder maybe become better. Thus, it is needed to avoid the administration of psychostimulants, which have recognized side effects. Moreover, having better sleep, we will see a better relationship between children and their parents and finally a rise in the standard of life of family members, which is a very important goal in our treatment. This review article evaluates available evidence on sleep medication in children with attention deficit hyperactivity disorder to present an appropriate guidance for this high prevalence problem.

Type A influenza viruses causes infections in human and animals, especially in birds. Wild aquatic birds are the natural hosts for all known influenza type A viruses. Avian type viruses are divided into two groups: highly pathogenic avian influenza (HPAI) and low pathogenic avian influenza (LPAI). HPAI virus is very dangerous, but LPAI virus is much weaker. Two forms of mutations including drift and shift have been recognized for antigenic changes in influenza viruses. Antigenic shift is responsible for producing re-assortment viruses with a potentiality to be transmissible to human and possibly resulting in pandemic. Emerging new species of viruses, the loss of previous immunity in human population and the transmission from human to human are the three major conditions that result in the occurrence of influenza pandemic in human. When pandemic happens, public health is a major concern due to probability of high fatality rate and other socioeconomic consequences.

Postpartum hemorrhage (PPH) is one of the most common complications of delivery and is a major cause of maternal morbidity and mortality. The aim of this report is to introduce a case of therapeutic uterine artery embolization (UAE) in a life-threatening postpartum hemorrhage. A 26-year old G3P3 woman with severe postpartum hemorrhage after 50 days of delivery referred to the emergency unit of an academic hospital of Mashhad University of Medical Sciences, Iran with third episode of vaginal bleeding. Gynecological examination and sonography were completely normal. Hypovolemic shock that was managed by fluid and blood replacement-therapy with uterotonic medical agents including oxytocin infusion (40 unit/lit), injection of 0.2 mg methyl-ergonovin and 1000 µg rectal misoprostol. Uterine artery embolization was planned due to unremitting severe hemorrhage. Embolization resulted in successful control of hemorrhage and the patient was discharged. At 6 months follow-up, no adverse events pertinent to embolization were noted. Therefore, UAE is suggested as a useful method for controlling intractable bleeding due to postpartum hemorrhage.

Introduction: Psoriasis is a chronic, inflammatory skin disease that is related to many genetic, and environmental factors, as well as infectious pathogens. Findings suggest that the Candida species, particularly Candida albicans, may play a role in the pathogenesis of psoriasis vulgaris. In this study, we aimed to systematically review the possible association between C. albicans and the prevalence of psoriasis. Methods: A systematic search of existing literature was performed in the PubMed, Scopus and Google Scholar databases and the Google search engine using the following search strategy ((Candida albicans OR C. albicans OR Candida)) AND (psoriasis vulgaris OR plaque psoriasis OR psoriasis) to find relevant articles that described a possible positive or negative association between C. albicans and the incidence or progression of psoriasis. The search was not limited to articles that were published within a specific time period; however, only those written in the English language were included in the review.Result: Of the 499 articles in total that were identified during the initial database search, 491 were excluded from the review because they failed to meet the inclusion/exclusion criteria. The total number of people involved in the selected studies, including both patients and healthy controls, was 1260. The analysis of the results of the included documents showed that the colonization of C. albicans is more prevalent in biological specimens taken from psoriatic patients. Conclusion: Studies show that C. albicans, opportunistic yeast, like diploid fungus, may be involved in the pathogenesis of psoriasis.

Chronic constipation is a very common disease that is particularly commonplace among members of the elderly population. It is one of the most widespread bowel disorders, and it causes significant pain and discomfort; as such, it usually requires medical attention. The major causes of constipation are slow colonic movements and/or functional gastrointestinal disorders. This review aimed to examine the pharmacological treatments that are currently available for chronic constipation. To develop insights into the causes and treatments of chronic constipation, relevant review articles that were published on the Pubmed, Cochrane database, and Embase websites, were examined. The outputs of these studies indicated that high daily intake of fibers and fluids in addition to regular exercise can be very helpful in avoiding and treating constipation. The pharmacological treatments that are administered to treat this disease typically increase the water content of the bowel lumen, and this leads to more regular bowel movements. Novel drugs have been introduced to treat constipation, and many of these are now subject to formal research studies. Since constipation can facilitate the development of other gastrointestinal diseases, it is important that we develop an understanding the therapeutic treatments that are available with the intention of identifying which of these may represent the most effective method for treating this disease. With that objective in mind, this review was undertaken to review the clinical effectiveness of the different pharmacological treatments that are employed to treat or prevent constipation.

Fragile X syndrome (FXS) is the most common known genetic cause of male intellectual disability. A wide variety of medical problems has been reported in FXS syndrome including seizures, facial abnormalities, macroorchidism, and autistic disorders. Here we reported a 9-year-old boy with fragile X syndrome that was confirmed through karyotyping and mental retardation. Initially, he was diagnosed as hypothyroidism when he was 15 months old. However, due to unusual clinical presentation, we re-evaluated the patient according to his history and clinical findings. Subsequently, targeted laboratory tests were performed and the results were indicative for thyroxin-binding globulin (TBG) deficiency in our patient. Therefore, levothyroxine was discontinued and one month later, laboratory tests were repeated and his diagnosis confirmed. As inherited TBG deficiency might also be X-linked, FXS and TBG deficiency may be coincidental findings in the patient.

Introduction: Nowadays, fake doctors, known as “Bandis” in our country Iran, represent themselves as traditional healers and use superstitious ideas for the healing of the ill, especially children. Their activities do not comply with any scientific methods. No research has been focused on this issue. The present study aimed to evaluate the education and social levels of the patients and their reasons for referring to these quackeries. Methods: This cross-sectional study was conducted on 1,300 children who were managed in the pediatric clinics of Dr. Sheikh Hospital for children, which is a tertiary care center in Mashhad, Iran. Informed consent was obtained from the guardians and patients. Upon examination, the parents of the children were asked to complete a checklist for data collection. Data analysis was performed in SPSS version 16, and the P-value of less than 0.05 was considered statistically significant. Results: In total, 1,300 children (621 females and 679 male) aged 2.5-12 years (mean age: 5.3±2.9 years) were enrolled in the study. Among the guardians, 62.5% were familiar with traditional healers, and 457 (37.2%) of the respondents had visited these individuals at least once. In addition, 106 of these cases had visited traditional healers more than three times. The parents were enquired about the reasons for visiting traditional healers instead of physicians, and 46.7% believed the applied methods by these individuals to be more effective. Moreover, 3.1% mentioned the lower treatment costs compared to physician visits, 2.9% reported the acute diseases of their children that did not respond to medical treatments, and 47.3% acknowledged those methods as real traditional practices. Conclusion: According to the results traditional healers, are a major public health concern. For unjustified reasons, many people prefer their methods to conventional medicine. Therefore, healthcare authorities must raise public awareness in this regarding through systematic plans in order to prevent future complications.

Coronavirus disease 2019 (COVID-19) is one of the most significant medical problems of the century. Mucormycosis infection is one of the devasting problems associated with this disease. Brain abscess associated with these conditions may have very poor prognosis, and prompt diagnosis as well as treatment of it is of paramount importance. In this study, we introduce a diabetic middle-aged man who was treated with corticosteroids due to COVID-19, and affected by rhinocerebral mucormycosis. Due to eye chemosis, brain MRI was performed which showed a silent abscess in temporal lobe with no neurological symptoms. After ten weeks of treatment, serial endoscopic sinus surgery and antifungal therapy, the invasive fungal sinusitis and brain abscess healed without debilitating sequala. We introduced this case as an educational example of brain involvement in mucormycosis. Although very rare, due to possible fatal sequalae, brain abscess should be considered as a probable complication of mucormycosis even if the patient has no neurological symptom.

In the present study, we aim to examine the association between depression, anxiety and stress with RLS, Epworth, insomnia, apnea, PSIQ and QoL by gender in Mashhad-study data. Methods: A total of 289 individuals (143 male and 146 female) aged 30-50 years old who completed the SUVINA study. They completed the Epworth sleepiness scale (ESS), Pittsburg sleep quality index (PSQI) and quality of life (QoL). For statistical analysis, SPSS software was used for data analysis and p value 0<0.05 was considered significant. Results: Depression scores were correlated with higher scores for RLS, ESS, insomnia, and PSQI in men but inversely related to QoL (P < 0.05). RLS and PSQI was positively correlated with anxiety in women, and subjects without anxiety had lower mean of RLS and PSQI scores than the other anxiety groups (P < 0.05 for both). QoL was conversely associated with anxiety in women. Stress was related with higher scores of RLS, ESS, insomnia and PSQI in men (all P <0.05).Conclusion: OlderAlthough scores for depression, anxiety and stress were higher in women compared to men, the association of depression, anxiety and stress with RLS, quality of life and sleep quality varies between genders and unlike our expectation these disorders were more highly correlated with RLS, QoL and sleep quality in men.

Vacuum assisted delivery might not be the first-choice route for delivery but remains a viable option when shortening of the second stage of labour is needed. Incorrect placement of the vacuum device, incorrect technique, prolonged use, and multiple attempts often lead to subgaleal haemorrhage, which is a collection of blood in the space underneath the galea aponeurotica. Here we would like to report a case of an infant that was delivered via vacuum assisted delivery after multiple attempts. The unfortunate infant suffered from subgaleal haemorrhage and was promptly assessed and treated. After spending 1 week in our center, he was allowed to be discharged home, with subsequent follow-up showing remarkable improvement. We would like to emphasize on the importance of this case because even though it does not occur very frequently, the consequences can be fatal if the condition is not identified and treated early.

Thus, the enhancement of chronic disorders and the development of the comorbidities become noticeable and evaluation on the cardiovascular risk factors in patients is necessity.Method: A systematic review of English and Persian literature was done by using electronic search in databases of Scopus, PubMed, Science Direct, Up-to-date, and ProQuest from 2000 to 2021. The study was focused on the all article types with the theme of the risk factors on congenital heart disease in adults. Initially, 200 papers were extracted and evaluated by the research team in terms of inclusion criteria; and at last 74 papers were selected. The association between adults with congenital heart disease and risk of cardiovascular disease by conducting a narrative was evaluated.Result: out of six groups of risk factors which were recognized for adults with congenital heart disease, the level of physical activity is highly impressive. The findings showed that while women were less active and more likely to be obese, male patients had a greater prevalence of smoking and high blood pressure. Although age is discussable specifically based on the other medical factors, sex was not individually determined for the self-effective risk factor. Conclusion: Cardiovascular risk factors remained the noticeable leading obsession across time in the world. Alarmingly, the increasing rate of adult CHDs require best healthcare investments.

Introduction: Various studies have assessed the link between the thyroid and kidney and concluded that dysfunction in one organ can disrupt the other. Thyroid dysfunction is more prevalent in individuals with end-stage renal disease (ESRD) compared to the general population. Hypothyroid ESRD patients have higher mortality than euthyroid patients. In this study, we evaluated the prevalence of hypothyroidism in dialysis-dependent ESRD patients and assessed the association of possible prognostic factors with mortality. Methods: Patients who were undergoing dialysis in centers affiliated with the Mashhad University of Medical Sciences were enrolled and followed for a year to obtain survival rates. Lab parameters including thyroid stimulating hormone (TSH), parathyroid hormone (PTH), and magnesium (Mg) were recorded at baseline, and the relationship between these values and mortality was assessed. Results: The prevalence of hypothyroidism was 23% and 32.5% in hemodialysis and peritoneal dialysis patients, respectively. Blood urea was meaningfully higher in hemodialysis patients, while hypocalcemia and hyponatremia were more common in peritoneal dialysis patients. Higher ages (P=0.006), lower baseline Mg (P=0.044) and PTH (P=0.01), and diabetes (P=0.037) were all linked to a higher risk of mortality. Conclusion: Hypothyroidism was notably prevalent in our study population. As hypothyroidism is associated with higher mortality, proper screening and intervention in this group are essential. We recommend the prescription of supplementary Mg in dialysis patients as baseline Mg and PTH levels are associated with better outcomes in this group. Diabetes was also associated with higher mortality. Maintaining glycated hemoglobin between 6%-8% is therefore suggested to increase the survival of diabetic patients.

Introduction: Gallbladder stone and sludge are common diseases worldwide. The causes of gallbladder stone and sludge are numerous. Since some studies have reported the effects of thyroid hormones on the function of the sphincter of Oddi and biliary discharge, the present study aimed to evaluate hypothyroidism as a risk factor for gallbladder stone formation. Methods: This case-control study was conducted on patients whose sonography indicated gallbladder sludge or stone and those referring to our clinic with pain in the right upper quadrant of the abdomen. All the patients were initially examined by a sonologist to confirm gallbladder stone and sludge. Afterwards, thyroid function tests (TSH, T4, and T3RU via radioimmunoassay) were requested for the patients, and the results were analyzed and compared with the control group. Results: Among 318 patients, 171 (53.77%) were male. In the case group, six females (8.4%) and 10 males (11.2%) had hypothyroidism. Overall, 9.9% of the patients (n=16) had hypothyroidism. In the control group, the prevalence of hypothyroidism was 2.5% (n=4) (OR: 2.082; 95% CI: 1.022-11.046; P=0.017). In the case group, 4.3% (n=7) had subclinical hypothyroidism, and 5.6% (n=9) had clinical hypothyroidism, while in the control group, only 1.9% of the patients (n=3) showed signs of subclinical hypothyroidism, and 0.6% (n=1) showed signs of clinical hypothyroidism. In the patients with gallbladder sludge and stone, the prevalence of clinical hypothyroidism was higher than subclinical hypothyroidism. Conclusion: According to the results, hypothyroidism was prevalent in the patients with gallbladder stone and sludge. Therefore, it is recommended that the thyroid function of patients with gallbladder stone and sludge be examined even if the patients do not harbor any other risk factors for the disease.

Introduction: The pandemic of COVID-19 created a psychological response. So, the psychological function of COVID-19 patients is an important subject that forces us to follow up with them.Aim: Assess the correlation between vitamin D serum concentrations and psychological functions such as insomnia, stress, and depression through the COVID-19 pandemicMethods: In this cross-sectional study, blood samples from 120 COVID-19 patients (61 males and 59 females) who had more than 30 years, were taken. Also, 25(OH)D Serum level of COVID-19 patients was analyzed. The Insomnia Severity Index (ISI), Depression anxiety stress scales (DASS), and the Short Form Health Survey (SF-36) were used to analyze insomnia, anxiety, stress, quality of life, and depression. Results: The relationship between temperature (p=0.039), PCO2 (p=0.022), and serum vitamin D level was significant. Additionally, there was a significant correlation between stress (p=-0.023, OR=0.389, 95% CI for OR=0.047, 0.843), depression (p=0.012, OR=0.659, 95% CI for OR=0.476, 0.913), and the concentration of serum vitamin D.Conclusion:This study recommends that vitamin D supplementation improve psychological state in COVID-19 patients.

Introduction: Metabolic syndrome (MetS) is comprised of a clustering of various cardiovascular risk factors that can also affect bone health. We aimed the associations between MetS and bone mineral density (BMD) and trabecular bone score (TBS) in subjects with abdominal adiposity. Methods: Individuals with body mass index less than 25 kg/m2 were enrolled from the SUVINA study and allocated into two groups according criteria of IDF for metabolic syndrome. TBS T-score and Z/T score of fore skeletal region were measured using dual-energy X-ray absorptiometry (DXA). SPSS software was used for statistical analysis and p value 0<0.05 was considered significant. Results: DXA measurements were made in 201 participants, of whom 75 had MetS and 126 did not. Scores related to the neck of femur and total femur, radius Z-score and TBST-score were lower in subjects with MetS (all P<0.05). Subjects with FBG≥100 had lower TBST-score (p < 0.05). SBP≥130 and DBP≥85 in subjects, led to the lower TBST-score, and lumbar T-score (both P < 0.05). Subjects with (LDL-C≥160 mg/dl) had lower TBST-score, neck of femur Z score, and total femur T/Z scores (all P < 0.05). TBST-score and scores related to neck of femur, and total femur were lower in participants with serum cholesterol≥200 mg/dl (P < 0.05).Conclusion: MetS is negatively associated with TBS and BMD scores. Higher levels of LDL-C and cholesterol were the most associated factors related to TBST-score decrease. The neck of femur was the most vulnerable skeletal against the MetS components increment.
